    I would suggest including a cover letter explaining why you are re-applying. I think this will be more important than the additional proof of residence documents (which they have from your previous RQ anyways). Good luck.

    This is not an issue at all. I had made the online payment then noticed errors in my residence calculation days due to travel. I applied a whole month after the payment date..no problems whatsoever.

    First off, sorry for your loss. I believe your application for a PR travel document from England triggered the RQ. A call in to CIC stating you'd be out of the country to avoid getting the oath invitation would probably have taken care of this. Look at it this way; an application from England...
