    Uploading Trouble HELP!! Form IMM1295

    I was having a similar problem with that form. What worked for me was to fill out the form in Adobe Reader (you may have to update to a recent version) and enter '00000000' into the UCI box on the first page, even though I don't have a UCI number. It won't accept letters so you can't write 'NA'...
