Attach the affidavit and the copy of the 10th certificate without notary

Depending on the lawyer you go to, he will / should stamp the copies of supporting docs (10th certificate) you provide with the affidavit. I just attached those docs.

You can go to any desi lawyer and they usually have a format. Just a note on 10th certificate, only submit 10th grade if you have your name, BOTH parent's name with DOB on the certificate.
