The best way is to send the form and passport to the Indian counslate NY. All the information is available in their website, you just have to follow all the instructions carefully. I did the same and received the PCC within the time mentioned. They do a great job and I found applying by mail...

I am not sure how to get notarization on some of the photo copies of certificates required like I-797/I20. I am residing in US.

I was asked not to use exact words from the NOC description. I wouldn't even write paragraphs as I guess the letter is usually one page.

Check with your attorney/Immigration consultant to see if you/your father can give an affidavit. The ideal thing it to attempt for changing...it might be tough but would be worth taking the effort. How about your Passport? What is written on that?
