Hi, glad to know you are working in the UK already. I just moved to the UK too 3 months ago from Canada ( after 2 years and 4 months) and working with the NHS. Looking forward to getting into GP training later this year.

Getting a spot for training in the UK is not dependent on whether you want to stay permanently in the UK or not. NHS do encourage retaining their GP trainees but it is not compulsory. A lot of doctors are in the UK for the training and planning to migrate elsewhere or return to their home country.

It is not compulsory to certify photocopies. Only translated documents need to be notarised.

Just use the updated form as it is.I think it is no longer necessary to specify the visa office. I sent my application with the updated form too.
