    On the application kit, there is a request form for medical and on that form there is a portion for a file number, will i just leave it blank?

    To start where can i get a finger print card aside from phil consulate? Its a waste of money if illgo to vancouver just to get one, please help. I need to submit my application before my permit expires on jan 2013... Plss help
