Hi all, I have a quick question: For residents of Malaysia, at the time of Passport Request (for Permanent Residency), where should one send the passport to (which Visa Office)? And is it possible to send passport via any courier? Any guide is much appreciated. :)

Thanks Drooza! But what is that "E" at the end of the Form Name (IMM1017 Form (05-1998) E). Does it mean it is an E-Medical? The visa office (Warsaw) has emailed the medical form (Section A only - one page) to my lawyers. So i am not sure if it s an E-Medical or any other type.
