You probably got confused with Visa and permit. The visa is the one stamped on your passport. It is attached with glue and cannot take it off unless you willing to make some damage to your passport.
On the other hand, the permit is a regular size paper (maybe letter size), which reads as Study...

I got my passport back today (May 09, 2012) by Fedex. The visa is stamped on April 27, 2012 (Friday).
My online status changed to 'Decision Made' on May 01, 2012 (Tuesday).

I got a strange situation.
PPR on April 20th by email. Send passport on April 24, 2012.
My ecas got updated today with 'decision made'. But inside, there is no line about "medical results have been received"
Now, I am worried as I noticed it should show medical received before decision...
