Alvin99, IMM 5688 is CoPR that Detroit consulate will be providing you once you give them your passport. Your details will be printed on a plain paper and you HAVE to show it while landing alongwith your passport. RPRF fee($490 per adult, for children you don't need to give) has to be paid now...
