Pankaj, I didn't mention vacation if it is less than 1 month, Its up to you, they just want to check if you have lived more than 6 months at one place then you shuld provide police certificate from that country. i guess this form is for this purpose only... cheers AT

Pankaj, Its better if you send bank statement for 2 months. Because my friend and me send bank certificate which mentioned about xxx person have xxx amount in his account. My friend got reply from CIC to provide bank statement instead of certificate. So my advice is to send 2 months statement...
