it seems that your file is already approved and transferred to Ottawa. Do you think you need to send Birth and Marriage Certificate now? If CIC did not specifically asked for these document and approved your file, I think you do not need to submit these document anymore...

I applied for my in-laws on April 2009 and received for additional documents from CIC yesterday...

Checked CIC website and found 'Decision made' status for my parent's immigration. Waiting for the letter to arrive and started PCC process...quite happy and excited...

I did not send affidavit for my parent's birth and marriage certificate. I thought that the DOB and spouse name in their passport would be sufficient. I guess I should get their affidavit done asap and send it to CPC. Any suggestion/comment?

I did fill-out the 'Form for Representative' as 'non profit'/ 'family member'. I am also sponser to my parents too. I call CIC to confirm and the lady suggested that it is fine. Nothing to worry, I guess...

I filled the 'Form for Representative' as I want to handle all the correspondance and quries with CIC. I've given my mailing address in the 'self-address label'. hope it helps...

If you never lived in quebec, you have to select No. As soon as you select NO, other fields will grey out. For Imm008, you have to validate the form before you print it out. When you print out, make sure the greyed out fields are evident in the print out.
