The stamp in your passport or the CoPR that you used to 'land' is not accepted PR ID - the PR card is the only accepted ID. However, you can apply for a PR travel document for that 1-time entry (Option 3) if you do not have (or you lost) the PR card as yet.

I'm not sure. You could try again or call to find out. Also, if you sent the RoPF with your PP you can call your bank to determine whether they have actually cashed your bank draft (or whatever you sent them) of the $490. This may confirm if there is activity on your file.
