For OCI renewal, do I have to SUBMIT my passport?

Do we received any UIC or other kind of identification number for submitting a PR renewal application? I did not reveive any.. nor did i receive an email confirmation. If I go to the PR portal, it just shows application submitted on 25th March.

My address on my driver's license has ON instead of Ontario.. does my application form need to have ON aswell... if I write Ontario, the application won't be accepted??

So submitting CRA notice of assessment for 2019, 2020, 2021, 2022, 2023 won't be enough?? I will also have to submit bank statements?? And for bank statements, it has to be for the whole year (12 months) of 2019, 2020, 2021, 2022 and 2023??? So minimum 10 (5X2) uploads? I'm so confused. Please help!
