Apparently on the last form, which is Consent and Declaration form we should provide parent name. By signing this form we also sign IMM5669 form, it is mentioned there. p.s. this is web application not PDF file

It is not pdf form but web based form. For pdf forms I did validate it and everything is good. I think it might be some website glitch

I also got my application returned due to IMM5669 form. It is Web-based form and signature part is missing there. I searched it and on some YouTube guide videos I do see that section where we should electronically sign it. But in my case that whole section is missing.
