Correct.its weird tho because it says family name as shown exactly on passport but if the sponsor who is citizen doesnt have passport and has only birth certificate,what do you put then?one thing im afraid to put the new last name because it wont match the birth certificate and the other if i do...

Correct .its just that i have done all the documents in her actual name......her birth certificate and everything is in her name but her ID..i only thought this would be a problem if i was changing my last name as an applicant since they care about pr card not the sponsor.

shes only assuming tho .her actual last name is still different.its weird to put her last name as mine in the forms since it looks like we are related....She is the sponsor ...So does it really matter?
