I would just write a letter explaining to them that the employer did not recieve any request and ask them to resend the request/let you know what documents are required from the employer. ..don't worry about it too much these things happen just do your part.

Oooh so sorry to hear,had she recieved medical request already? I believe you can appeal by sending them a webform and explain the situation tell them you sent the documents requested and proof of ability to perform the job..Mistakes do happen and if you feel it's a mistake write them a letter...
