Your best bet is to apply for surrender certificate + entry visa at the same time. There is still time for this. Too late for OCI. I would say dont try to travel into india with indian passport and then try to exit with canadian passport.

Go through the check list again... no where does it ask for original passport..

Can't help you with the questions... but I am in exactly the same situation... same date of acknowledgement as you.. same travel date... same Mission and same dilemma about visa application... I have an appointment booked for visa for 21st October... but I am not yet sure if I should attend it...
