Hi all... I just completed the Schedule A Background form and when i clicked validate it says it has been validated successfully but on the first page in to right corner in the little box that says OFFICE USE ONLY it says Validated NO

It specifies that it should be scanned in colour and uploaded... mine is black and white, am I missing something? Also should I upload just the actual letter/report (second page) or both the first cover page that has the big logo and name/address info on bottom and the letter?
