Hello , I have recently downloaded the IMM 5257 E form however the box that indicates the type of visa requested does not show if it is SINGLE/ MULTIPLE entry. It only shows visitor/transit. I would like to have a multiple entry visa, however i do not know how to indicate it in the form. Also...
