I've been working in Montreal (as a physician) for the last 12 years and I was assuming my application would be easy to evaluate because of that, but after 14 months after my application has been received in Buffalo, I had no news other than the acknowledgement of reception in October last year...

They received my MR in October 2011. I actually don't have my passport, but because I had to send it to renew my visitor / worker visa. So far I wasn't requested to send passport for my PR (and they have received my application in May 4th last year).

I applied as Quebec selected skilled worker and since May 4th 2011 it appears as "received" on the CIC site. In October last year, I received an e-mail asking for the Medical exams, that I send the same month (and they also appeared as "received"). As my application has been sent to Buffalo...
