contact your visa office before proceeding, they might ask you to use the same form. All the best!

Hi, I had a similar case with my daughter's medical. I contacted the London office and they advised me to proceed with the same form and the mistake would be amended on their system. So nothing to worry about really, but I suggest you send them a mail before proceeding. Good luck!

Hi Biblo Thanks for your mail. Well I believe haven been selected already by Quebec itself, they checked my application before issuing the certificate of Quebec. So I don't know what the embassy still needs to check apart from the medical and police certificate that Quebec did not do.
