Check their book online test, it will show if you are already eligible for G1 or G2 exit exam.

I used CPC - Vegreville, AB. Below is what I have from the CIC checklist/letter. The forms and documents for your dependants must be submitted within 90 days of the date of this letter (see date above) to: Case Processing Centre – Vegreville Unit LC2 6212 - 55th Avenue Vegreville, AB T9C 1W3

If you are applying under LICP, for question #2: Live-in Caregiver Program. Question #4, usually in Vegreville, AB. See your checklist emailed by CIC, it shows which processing center is requesting your additional docs.
