No need to worry, i did that mistake and sent them email with the updated form having correct dates. It took 7 days to get the reply.

Hi, I mistakenly signed attestation form on the date of application submission instead of EOI submission date. I put EOI submission date correct in the form but I signed it on the date of application submission. Would that cause any rejection? What is the possible solution for that please guide
