family information form[IMM 5645] is mandatory and if some other family member is accompanying you then you have to fill "Schedule 1 – Application for a Temporary Resident Visa Made Outside Canada [IMM 5257 - Schedule" form for each member.

Hello raquibahmed Salary statement is what u got from ur employer normally in the mails. If u are not receiving salary statements in mail, then plz have ur salary amount on the company letter pad signed by the HR or someone authorized to do so.
