You don't have to send any form for eye, etc. It is the responsibility of DMP to send your all the medical reports to CHC. Thats why you pay fees. Just attach cover letter, your receipt of medical fees, Letter of Medical request, passport request, Landing Fee request (if any) with DD and...

Medical is done already and passports are sent with Medical. I already got RPRF receipt before medical when i got second AOR. Just wanted to know that does CHC send passport immediately after "Decision Made" or after some time? There are already 3 days passed after status "Decision Made"...

That was already paid at the time of sending full docs to local CHC and also got receipt with second AOR.
