my wife applied her LC1 in Canada last April 2010

we applied our LC2 last November 2010 we received our file number last December 2010 we received medical request and done with it March 2011 received letter of visa approval and request for passports May 31,2011 sent passporst thru DHL June 1,2011 Received passports with visa thru DHL July 26,2011

please help our family name in our visa is incorrect.
