I've been married for almost 6 years, never bothered to get my name changed. All my legal documents still have my name with my maiden last name. Too much hassle to change everything. I don't think it should impact our sponsor application though.

If I am only sponsoring husband, and no one else, does he still need to fill out that form for additional family? He has no other children, (only two with me, and our children are Canadian in Canada). His mom and brothers are still alive, in his native country. Thanks in advance

Maybe CIC had cases that certain applications were printed and reprinted from different printers? Not everyone owns a printer and they might have had friends or hired agents handling the papers.

I think you can ask your current employer for a letter confirming your job and income.
