What do you mean? The way the medical exam works is that you do a chest Xray and some other tests then the doctor sends a copy to you and one to the immigration medical center which is valid for a year. You must not re take the test even after a year unless they officially ask you to re take...

I think you have to post them the form and make sure to have it sent by registered mail so you can keep track of when they've received it, so if they claim that they never got it (which is code for: they've lost it) you can refer to the tracking number and the postal service's website.
