Best to either extend your work permit or apply for a SOWP. The processing time was 9 - 10 months when we applied in February but individual timelines vary quite a bit. I've seen people get theirs in 4 months while others its more than a year.

I would suggest following up with IRCC about the OWP. We applied February 23rd and received the permit within a month. Not sure about the medical exam as IRCC can be funny these things. I'm still bitter about having to redo biometrics when my current one is still valid.
