No, BLS did not ask for the affidavit at all. And no affidavit was provided in the past.

My wife's and daughter's passports have Bombay as their place of birth, whereas the Canadian passports lists Mumbai as the place of birth. They had no issues with their OCI applications. Applications submitted at BLS Mississauga this June.

Do not add "Late" to their name. Write the name exactly as shown on your surrendered Indian passport. Write their occupation at the time of their death. If they had retired, write retired. My dad was retired at the time of his death. So I wrote "Retired" in the occupation field.

Two of my applications (daughter and me) were processed on 22 July and dispatched the next day. The remaining two (wife and son) were processed on 23 July but haven't been dispatched yet.

Each to their own. But I unstapled mine and have kept it in a sheet protector separate from the passport .

E-mail the Toronto Consulate OCI section at oci-toronto@mea.gov.in and ask them to clarify regarding the marriage date/marriage registration date question. I e-mailed them once regarding some other question I had and received a reply the same day. And attached their reply along with my application.
