I did submit supporting documents just to be on safe side. I would suggest to do same.

I specifically asked vfs guy to have my application under CAN+, and he attached some kind of form along with my documents and that form has one checkbox saying CAN+. Regarding your question about visit USA, I think by just having USA visa gets you entry in CAN+, not sure though.
