In the address history, only put your permanent address for Sialkot and provide the PCC. You don't need to provide your Isb and Abbotabad addresses.

If you are only using 1 address in your address history, and that is the Lahore one, then no need to get the one for Attock. Ideally one should get PCCs based on the permanent address, but its fine if you don't. I generally recommend to get a single one for people who have moved multiple times...

If you make a PCC to date, that will cover everything in 1 line and should be fine. I don't think you need a separate line on the PCC for every date. That would be too much.

sorry for late reply on this but your address in address history and PCC should match. If your permanent address was in Karachi while you worked in mianwali, just use the Karachi address in address history and use its PCC. Remove the mianwali address.

You just need to report one address. Just report the permanent one and get the PCC for that. Although if you get both PCCs that is fine too. You can definitely be living in 2 places during the same period of time. Should not be a problem.
