So I only can do (Passport/health insurance card) or (PR/health insurance card). Apparently I can't use 1. Passport and 2. PR. Of course my passport and PR have photo on it. But I'm curious if it's ok to use without photo for health insurance card.

I think this is for certificate. Not application for citizenship.
