I received my notes in a shortened format. Not much info there. I was not aware that they changed the format of GCMS notes and we can request the older format. yes, the application assignment date was on the first page, exactly as you mentioned. I applied for the notes on 18 Aug 2021.

Thanks for replying. I am not able to locate the GCMS notes generation date on the notes. But I received my notes today, so we can consider 26 Sep as the GCMS notes generation date. Also, just curious, if you know what they mean by application assignment date. In my case, it was 24 June 2021?
