You dont need consent form please. it is only needed if a representative is requesting on your behalf or if you have dependent and need information about the dependent to fully reflect on the note being requested.

Sorry about your experience. Come to think of it, what field of study is it that led to you being in SS? As for CSIS, I think it take pretty longer than CBSA/GCMS notes. I heard it could take 3 months or even more if I'm not mistaken. I applied mine over 2 months ago and yet to get a reply...
