Is it mandatory to self attest the photocopies ? All of you attested photocopies? I haven't done for any. I don't know what will be purpose of self attesting here.

Why did they ask to fill it again? Something you missed or its random?

"Just make sure you attach the return courier receipt somewhere in the application package." I only got payment response which does not mention any return receipt. I think this process is way too complicated then it should be =) but what do I know.

Did this resolve for you ? I am curious why would one centre ask and the other one doesn't for ITA.
