You can take the print out of the form. Fill details like name, signature etc. And at places where you know the space is less, just write document attached. Then make a table in the same format with the same headings on Microsoft word. Put the question number on top of the table and also details...

Oh wow! thanks for pointing out. Just corrected it :) My initial medicals were bound to expire in Jan 2020 but I received the re-medical request on 10th October 2019. Maybe because of the additional documentation they required, not sure though.
