Form 5669 Validate or without Validated

Do I need to fill out IMM5669 with a validated or without a validated form? There are two 5669 forms one is eIMM5669 (Modified 11-2018) with validating and another one is IMM5669 (Modified 06-2019) without validating.

You should download the forms from IRCC website directly. Overthere you will get the most recent version of all forms, and that’s what you should fill.
