My wife is pregnant. My mother in law has visitor visa. They came in 2016.
Now wan to come again for my wife's pregnency.

Is there any travel restriction ? Do they need any letter from doctor ?

If her daughter is a PR she can enter Canada with her valid TRV. She will need to do the 3 day hotel quarantine and will need to have a separate location to do the rest of her quarantine because pregnant women are at higher risk of getting severe covid. A separate basement with bathroom or AirBnB would be options.

My mother is travelling alone to Canada first time and I m citizen of Canada. Just wondering how does transportation from airport to hotel where she needs to quarantine for 3 nights works. Can I drop her from airport to hotel?

What city and hotel? Hotels have shuttles from the airport to the hotel. No you can’t pick her up from the airport and drop her off at the hotel. That defeats the purpose of mandatory quarantine.

I have not booked hotel or airline yet. She is visiting me in Toronto around last week of May.
She is travelling internationally first time so little worried about all these hotel quarantine protocols. So I was thinking to pick her up from airport and drop her off to the booked hotel.
Do you know if wheel chair assistant can help her drop off and help board hotel shuttle in this case?

You can’t pick her up as that defeats the purpose of mandatory quarantine as she is Covid tested on landing and has to wait for the results in her hotel room. Now you can ask to pick her up and then you go into quarantine with her for the three days and undergo Covid testing and then 11 day quarantine with her. Not sure if people have tried it and paid for it. If you are willing to do that can’t hurt to ask when booking the hotel. If not, you don’t pick her up at the airport.

Don't know if there will actually be wheelchair assistance to the hotel unless she has a true disability that requires disability transportation which could likely be arranged. You would likely have your own wheelchair if that is the case. There are limited people to help disabled people and wheelchairs at the airport so unless she is truly disabled she should just ask for help getting through the airport. There are plenty of people who are available to help at airports without potentially using a wheelchair that someone may actually need. Would add that because your wife is pregnant even after quarantine your MIL must remain separated from your family for the rest of the 14 days because pregnant women are considered more vulnerable to covid and they also have more contact with the healthcare system. She will need to stay in something like a basement if you have one with a washroom or an airbnb.
