There is a option:
Spouse or common-law partner

Here I should fill my husband's name because he is my sponsor??????

IMM5490 question 30 and 31

1. Why when I press YES in the question 31 (about my consent to disclose), automatically it made delete the previous question, number 30?

In the question 30 I fill the quantity of the the options: How much/ How often.

but I don't know why it is removed, when I fill the question 31.

Sorry for again ask...but really I will very grateful.

I am the sponsor (canadian citizen) sponsoring my husband. So in this form IMM5406E, my husband puts his name, then mine as in the section spouse/common law.

I have one more question or you guys regarding the IM 0008, we have no dependants, just me sponsoring my husband so do I leave this empty? there is an option to delete dependants from this form?

Additional Dependants/Declaration [IMM 0008DEP] (PDF, 434.37 KB) - Do we have to fill out this form then? Or just disregard it?

You have to fill IMM0008 form (http://www.cic.gc.ca/english/pdf/kits/forms/IMM0008ENU_2D.pdf) and as you said you do not have any dependent then you will discard the Dep section on that form.

In the additional family information form. When hubby is applicant and i being the spouse (also sponsor) is being mentioned.

Now do i also as a spouse and sponsor have to fill up a seperate additional family info form????

And for deceased parent. Its asking city country and date??? There is already dob column. What other date and place are they asking?
Place where they passed away and date when the passed away????
