Hello Everyone,

I am having trouble completing the application for my mom. In the applications it does ask for Given Names. In the past, she has been approved and granted two visitor's visa from Canada; all of her previous application for Canadian visa and granted visa only indicated her Surname and given name. Her previous applications does indicate the middle name because there was no spot for it although it is clearly shown on her passport.

Her passport which was issued in the Philippines it shows the following:
Surname: Mercado
Given Names: Milagros
Middle Name : Mores

When I applied for the interest to sponsor form. I did not include her middle name and only indicated Mercado, Milagros. If I include her middle name this time,I am afraid that if her application does not match the interest to sponsor form they will reject the application for PR application. I indicated her UCI number in the PR application so CIC can refer back to all her previous applications which will only show as Surname as Mercado and Given Names as Milagros.

Please let me know if I am still okay to put only her Surname and Given name as shown on her passport and If I should just attach a letter of explanation in her PR application.

Any feedback would be appreciated. Thank you very much in advance!

Include her middle name in the given names. It should not be a problem. In Canada, first and last names are more important, as long as they match, should not be an issue.

Hello Again,

I have a couple of question regarding the current country of residence and previous countries of residence on IMM 0008.

QUESTION # 1 : Mother is currently in Canada as a visitor (Super visa). She entered Canada on Dec 28, 2018, interviewed by the border officers and was allowed to stay until Dec 27, 2020 as indicated on the stamp in her passport.I was informed by IRCC that since she is currently in Canada we would have to put Canada as her current country of residence and her status. I did ask them as to what date will I put into the "TO" section and did not really get a straight answer. I was told a couple of things:

1. I have to put the date I am submitting the application and attach a letter of explanation why.
2. I could not really put the date she intended to stay there might be changes in the future.

Although my mother intends until the date she is allowed to there might be changes in plan because we plan to go back to the philippines with her this December and come back in Canada after a couple of weeks. Would putting the date I submitted the application with this explanation be an ideal thing to do?

QUESTION # 2:
Previous countries of residence question # 12 ask "during the past five years have you lived in any country other than your country of citizenship or current country of residence (indicated above) for more than six months?

My mom has only lived in the Philippines (her country of citizenship) and Canada (current country of residence) she has stayed for more than six months in Canada as a visitor in the past. Would the answer to this question be "NO?"

Any feedback would be appreciated. Thank you very much in advance!!

Thank you for your response. If I am putting the date she is allowed to stay until in Canada, do I need to give a letter of explanation to explain the "to" date? Please advise.

IMM 5669 E SCHEDULE A BACKGROUND requires validation but does not produce a barcode? Is this how its supposed to be or my form is faulty?
