Hello, I have a question on Form IMM 5748. Under 3/(e) Amounts paid to you under the Employment Insurance Act, other than the special benefits. Is that the amount on line 119 of your tax return, i.e., the money you received through EI when you lose a job.

Thank you so much for your help. What would be the "Family size " column at question 14 on Financial evaluation form: - I am assuming it will be my family member(myself, my wife and my kids) excluding my Mom??

Always include parents in family size.
Guide 5772: You, the sponsor (and your co-signer, if applicable), must prove that you have enough income to support all the people you’ll be financially responsible for once you become a sponsor. We call this your “family size”, which is the number of people you’ll be responsible for.

I was looking at the 2018 tracking sheet and noticed some people were asked for additional documents for travel history. anyone knows what it is for? anything we can include upfront? I did not see any request for travel history in the forms or checklist.

You can provide copy of visa stamps and copy of pages on passport which shows visa stickers.

Hi, my mother is on super visa with me in Canada. can I quote her UCI number and application number from that application or is it ok if i send it as blank ?

It will be her UCI number forever so use that UCI for this application as well.
Application Number should be N/A

Hi,
I am PR in Canada and received Invitation to sponsor Parents. I have a few questions about dependents and birth/marriage certificate requested by CIC in the application package.

1. I have a younger brother (21) and sister (27). Can I include both of them in the application or only dependent who is less than 22 years of age?
2. I (sponsor) don't have a birth certificate. Is there any other document that I can provide to prove my relationship with my parents?
3. My parents are in India, and my father (Principal applicant) doesn't have a birth certificate and marriage certificate. What other documents can we provide to prove their Identity and relationship?

As said a couple of times, write the full name on the computer, and after sign by the side of the name

Is that how you all did it?

I was able to validate it by just typing the date. Don't know why major of people here can't validate the form without typing the name which is not necessarily needed to have the form validated

For those not yet sent out the apps, here are some common mistakes to avoid which may results in your application being returned:
1. Sign and date using blue ink instead of black ink
2. Forgot to stamp at the back of the photo or missing name, DOB, name and address of photographers or self printing of photos
3. Wrong order of documents, put the barcode page within the application pages or separate bar code pages from the front and put in front of each form, barcode pages must be together and right after the check list
4. Send to Mississauga instead of Ottawa PO Box
5. Include more documents than requested including original documents that must be returned
6. Not paying the correct amount of fee
7. Faked signature of parents
8. Signatures are not within the box
9. Not filling the name in the signature box in the form with e-sign
10. Barcode pages are corrupted or not readable
11. Write NA in field that are required or fields that are supposed not to be filled
12. Give SIN number and also send the form that say FILL only if NOT authorized to release tax return causing confusion to officer on what action to take whether to pull Tax return directly
13. Not include invitation letter
14. Using different pages size (A4 and letters mixed up etc)

#9 handwritten signature is the one required not the e-sign

IS IT REALLY REQUIRED TO TYPE THE NAME ON THE SIGNATURE BOX TO SIGN ELECTRONICALLY ON IMM5669? HANDWRITTEN SIGNATURE IS THE ONE REQUIRED AND I DONT KNOW WHY SOME OF THEM CAN'T VALIDATE THE FORM WHEN YOU JUST HAVE TO SIMPLY TYPE THE DATE TO SUCCESFULLY VALIDATE.

Hey guys, i am in process of filing application for my parents and my brother who is dependent, so application for 3.

How many hand written signatures required in complete application? I think me and my father has to sign the sponsor application, then Sch A has to be signed by all 3 sponsored. Is there anything else i am missing?

The other question is, after doing the tax return for last year i owe some money to CRA, If i don't clear the dues with CRA, Will this make my application rejected? I am trying to clear the dues before i submit but its not small amount.

Really!!!! Which IRCC invitation letter will work? I think your application will return from MIDI . You can not apply without checklist point 2 in form #1520.

Following information confirm with Immi. Quebec CSR via phone support:
You have to wait for IRCC acceptance letter before going to MIDI. you can not submit to MIDI with IRCC Invitation letter.

14. Provide your employment history as well as periods of unemployment for the last THREE consecutive taxation years preceding your application

1. I should use NOA for year 2016, 2017 and 2018. Is that correct?
2. I am employed since 2015 with same company. how should I input my gross income? should I update only one row with total sum of income from NOA (line 150)? or should I enter 3 separate entries for each year?
3. what should be From and To date? Jan thru December? or financial period (i.e. March thru Feb)?
4. The address and phone number is my work address and work phone number or my company head office address and phone number?
