i received an email from scotia bank, which has a welcome letter attached and a pdf which says INVESTMENT DIRECTIONS CONFIRMATION

is the investment directions confirmation called the GIC

Do I need to fill out any blanks which are left in the Investment Directions Confirmation after taking out the print out?
There are two pages, one which needs Signature and date whereas the other has an agreement to be signed and it says some "Additional comments for instructions made by Tel/Fax/Email".
