I received ITA in this draw with a score of 434.
I am the principal applicant along with my wife.

I received a letter in cic mesage tab mentioning the ita and that I have 90 days to complete the submission.

I contacted the medical centre for getting a appointment, they messaged me the list of requirements like copy of passports and photos which is fine but they also want IME ID number (application form).

I am not sure about this.
Please tell me what should I do to proceed.

Tell them it's an upfront medical exam for Express Entry and carry a print of your ITA letter with you.

Please only share information that IRCC asks for. No need to complicate matters by providing unnecessary details.

1. Only 10 years.

2. Only Bachelors and any other degree after that.

3. Yes. Doesn't matter what you write. Write whatever you're comfortable with.

4. No issues. Write LOE.

5. No. Write another LOE.

6. No. Just stick to the IRCC format.

7. If you haven't changed your name officially and your name just appears differently in different places, select "No" and upload the same name affidavit.

8. You can include transcripts if you like but they're not required. Include IELTS certificate and ECA though.

9. I've no idea.

For 7, you mean to say mentioning one's name differently should not be considered as a legal name change?

Changing your name legally is different than mentioning the same name differently in different documents.

In your case, it appears as if you haven't changed your name legally, it's just that your name as been truncated in some documents and elaborated in others.

You need to tell them you need "Upfront Medical" and category is Worker/Non-EDE
I got it done a week before I got ITA. There is no need for ITA letter.

In your case, it appears as if you haven't changed your name legally, it's just that your name as been truncated in some documents and elaborated in others.

I have few queries regarding the details that I entered in work history.

I am Ph.D. in Chemistry and my first job started as soon as I submitted my Ph.D. thesis and I worked in that job till my Ph.D. viva after which, I was promoted to my second job in the same institution.

Despite having all the necessary documents to prove the authenticity, I am skeptical about CIC considering my first job prior to receiving the Ph.D. degree.

I could have shifted both the first and second jobs from work history to personal history and kept only the third job which is sufficient to get max points (3 years). However, somehow I didn't feel like doing it and have kept all of them. Please let me know have I done the right thing.

Also, I have mentioned that date I got qualified to work in this profession was the date I submitted my Ph.D. thesis cause my first job required to have Ph.D. thesis submission as a minimum requirement. So, Please share your views on this too,

Amit, have you already submitted your application? If yes, there's no point in worrying about these things now. These are minor issues that won't affect the outcome of your application. Good luck.

I have just received my ITA on 6th of this month. I m yet to submit my application. So, in that case should I make these changes? All the questions are related to my EE profile.

OK, in that case keep all the jobs that you've strong paperwork for in the work history section - as long as you have reference letters and pay stubs there's no reason why CIC will not consider them.

Secondly, you can put any date you are comfortable with and believe is the date you got qualified to work in your profession - there's no right or wrong answer for this one.

In a nutshell, don't overthink. Just follow CIC's instructions and fill in the application as correctly as possible with strong paperwork and you'll be fine.
