Hi mrs stylz..i have the same case,i am thinking that our app is missing..my husband send our app on may 3 and was received on may 4 but until now we haven't yet received anything and still waiting for the AOR..how can i request for special inquiry about this? I have called a couple of times but the agent said they dont see any files of ours in their system..we are really worried now..please advise us what can we do just like what you did in your app..thank you in advance

What Schedule A form is everyone using? I see that there's two different versions. I selected the June 2017 version but it states "Only to be used with kits 5289 and 5772". I have no idea whether or not that's applicable to me. Is there a form from another date that I should be using? They appear to be basically the same thing at a glance.

I got AOR on Jun 09, i havent got Schedule A request or PCC. i got medical request though. on my GCKey, it says "we do not need additional documents".

I haven't got medical request . but yeah my GCkey says "we do not need additional documents". this is so weird.
