I am applying for my parents sponsorship
My father has recently got stent and has diabetes and health issues. Alhough he does travel countries and has visited Canada recantly

Please let me know if there would be some problem getting the medical cleared for him? What kind of check do they do and could unhealthy heart be a reason to deny PR

Both an unhealthy heart and diabetes can be reasons for failing the medical and being refused PR for parent sponsorship. It all really depends on how serious his medical conditions are and how much CIC believes he will cost the health care system over the next 5-10 years. If CIC feels the cost of his care will exceed $6,500 per year over the next 5-10 years, you can expect to receive a fairness letter where you'll be provided with a chance to present arguments to the opposite before CIC makes a final decision in your father's case.

it depends on what health condition he is in
my dad had a heart surgery 10 years ago and had diabetes for 20 years but he is in a general good health otherwise (no expensive medical needs) so he passed the medical
the panel doctor requested extra heart reports, scans and echo, and a history of the health case, and some other tests too. the panel doctor has a list of medical tests required depending on the case so he should let you know what is needed
when I requested the GCMS notes, it stated that: he has a condition but it was not expected to cause him to fall under the excessive health demand condition.

as per Scylla's explanation, if they suspect that it will cost money in the future (more than $6,500 annually for the next 5 to 10 years) then they will refuse so it depends on his overall general health condition
