Hello everyone I really want some help!!! I applied for PNP and I got a letter from the immigration asking me to provide details about schedule A background declarations :

Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide the day/month/year and ensure that you do not leave any gaps in time with all sections updated to today's date.
> This must be received at this office by: 2024/05/31.

I have provided all details in the original application, however, they ask me to provide full date ( day/month/year ) and the schedule A background declarations IMM 5669 doesn't support full date format and what the version of IMM 5669 i have to use IMM 5669 (05-2021) E or IMM 5669 (05-2021) E

What do I need to do, please ?

The full date is required when it's indicated as YYYY-MM-DD

there is no place indicate the date as YYYY-MM-DD format in the IMM 5669 ! However, do I need to print out the IMM 5669 form and put physical signature ? or electronic signature is enough ?

There are lots of places to do that like dates of birth and death, check the form

And yes. you can print it to fill out.

Yep, it looks like they ask for the original signature, so you need to print it out and then you can use the required date format anywhere.
