re entry visa

im working as a caregiver here in ontario and i have my open permit.im going home this coming july and they told me to apply for a re entry visa so there will be assurance that i can come back and work here in canada if i go for a vacation in philippines.i dont know where and how to apply for a re entry visa..will anyone help me pls and thank you
