IMM5562: Supplementary Information or Supplementary Information Form for Pak

I have received a new message in CIC account on principal applicant side to provide the below:

IMM5562: Supplementary Information - Your Travels. Please submit a properly
completed Supplementary Information Form for Pakistani Nationals. Specifically, the Personal History
section must not have any gaps and must cover the required period of time which is:
-for males: the last 10 years or since you turned 15 years old, whichever period is longer
-for females: the last 10 years or since you turned 18 years old, whichever period is longer

Is there anyone else who has provided this form? We have already provided the travel history for the principal applicant in supplementary information. Moreover it seems we should not include trips to native country (Pakistan) and residence country (UAE). Anyone who can guide me about this?
And do we need to send IMM5562 only or we have to submit supplementary information form for Pakistani Nationals IMM3907 which has personal history or both.

It sounds to me like you should fill out the travel section as if you were living in Pakistan, in which case all trips to your country of residence should be listed.

I would send both. It is not clear to me that they are asking only for travel information.

Thanks for your reply.
And yes I am really confused if have to send one out of this or both. If I fill the both specially IMM 5562 then there will be effect on other forms as well like back ground declaration Etc as IMM 5562 is more detailed than county specific supplementary information. Can I put a case specific inquiry to visa officer regarding this?

You could try.
It sounds like the visa officer specifically asked for IMM 5562 in the letter though, in which case you must send that one in. If there is some detail that was not included on previous forms, you can attach a letter explaining things.

Hi,
My mother (principal applicant) has been asked to submit Supplementary Information form for Pakistan Nationals. So shes submitting that form only.
Not sure if this comes in the background check!
