Outland Spousal Sponsorship Question - Forms

Hi

Sponsoring my spouse (outland) and had a few questions -

1. Is IMM5490 still a required form? I donot see it on the Online PR portal through GCKey
2. IMM5406 for me the sponsor - this is the additional family info form. Do I need to have that added ?

Are there any other forms that I should be adding ? I have everything the checklist asks for.
